Community Health Workers

Community Health Workers are trained to assist community members in navigating the complexities of their healthcare system. They work to identify and reduce barriers to accessing healthcare. Typically they share similar life experiences or cultural backgrounds to those they serve. Some Community Health Workers assist clients through the screening and diagnostic phases of treatment, while others may help patients with the ongoing challenges of living with a chronic medical condition. Community Health Workers may also help to link people to financial assistance for a variety of needs including housing and transportation.
